Infant Enrollment Applications (Fill Online)

a monthly tuition fee. The tuition fee is due on the indicated day of the month. (Late payment charge of $30 will be added for payments made 3 days late). I understand that this amount covers expenses for attendance of my son/daughter to Baby Steps Daycare Inc, use of all programs, educational instruction, supervision, educational material toys, participating in all daycare center activities, daily meals (2X a day, and snack). This amount does not include any trips outside the center, except a walk to the park (permitting weather), hospital fees’, or any medicine other than provided by standard day care center emergency procedures.

I understand that there are no deductions for any absence in case of illness, vacation (half a time due), or other reasons. Full payment is due regardless of government or religious holidays included in the School Annual Calendar.

I understand that the Vacation Policy are as follows (1 week of vacation minimum to apply):

Requires at least 2 weeks notice prior to receiving the next invoice.

$30 charge, if advised after receiving the according month’s invoice.

I understand that my application deposit is refundable up until the day that a spot is offered by Baby Steps Preschool. Should I decline the spot, the deposit is no longer refundable.

Any incoming parents must submit a $300 deposit; refundable at the end of enrollment/aging out of the infant program only. If a family pulls out and does not give at least two weeks notice, the deposit is held by the school.

Baby Steps Infants is open Monday - Friday, 8:00 am-6:00 pm. If my child is picked up after 6:05 pm, I am responsible for paying a late fee of $2 for every 1 minute late, if advised, $3 for every 1 minute late, if not advised.

For the safety, welfare, and proper maintenance of all children of Baby Steps Daycare II, retains the right to terminate the Contract for the following reasons:

The child’s behavior is destructive, uncontrollable, violent, or threatening to other children or providers at the facility’s care.

A parent refuses to pay tuition to BSDC INC.

In case of an emergency, Baby Steps Infants will take all the necessary precautions to keep the child (ren) in good health, including but not limited to transportation to an emergency room. The Parent(s) agree to pay all costs and expenses incurred in connection with any medical care provided to the child. I understand I will make all the effort to contact my emergency contact or myself before or immediately after such emergency treatment is rendered.

I hereby confirm that the above-named child is in good physical condition and has been examined by a physician within the past 6 (six) months and is in relatively good health and able to participate in Baby Steps Daycare Inc educational and sports programs.

As per NYC mandate, all students must receive the Dtap, poliovirus, MMR, varicella, hepatitis B, and yearly flu vaccines. Baby Steps Preschool requires a doctor's note if your child has been diagnosed with strep throat, pink eye, influenza, or any other communicable disease. Without a doctor's note stating the wellness of your child, we cannot allow their attendance back to school. (Please understand this is for the consideration of your child's classmates and teachers' well-being).

Permission hereby is granted to Baby Steps Daycare II, to take children out on community walks, and or trips, informed via email to all parents a week before the trip.

Baby Steps Infants is not responsible for any personal belongings; clothing, shoes, jewelry, toys, etc. Please remember to LABEL your child's individual belongings. We reserve the right to discard any soiled clothing due to sanitary purposes.

Parents must notify Baby Steps Infants in writing of any changes of address, telephone numbers, emergency contact not later than 7 business days after the changes.

Parents are entitled to visit Baby Steps Infants at a scheduled time and watch their child for a certain time as long as it does not interfere with normal activities and does not distract children’s attention.

I have read the Agreement of the Enrollment terms and agree that this enrollment is acceptable to me and is subject to everything contained therein. In the event one parent executes this agreement, I acknowledge that I am also acting as the agent to the other parent with the authority to enroll my child into Baby Steps Daycare Inc and agree to execute this agreement on his or her behalf. I recognize that Baby Steps Infants Inc relies upon the representation herein made in accepting my child to Baby Steps Infants.
